Do I qualify for the house calls service?
All seniors on Medicare, Medicaid, or Medicare Advantage are allowed to have their medical care delivered via house calls. No longer does the reason for the house call need to be medically necessary.
Can Dr. Cook be my primary care provider?
The short answer is yes, if you qualify as a permanent patient.
While we would welcome any patients wanting to make us their medical home, not everyone needs permanent in-home medical support.
Patients can enter the practice as a:

- Patients with temporary disabilities
- Dr. Cook acts as a consulting provider working in coordination with your primary care provider to get you back on your feet.

Do I have to change my primary care provider to you?
The short answer is no.
While we would welcome any patients wanting to make us their medical home, not everyone needs permanent in-home medical support. We can support your recovery and communicate with your primary care provider in the meantime.
