Transitional Care Management
Dr. Cook sees recently discharged patients within 1-2 weeks of discharge from a hospital or rehabilitation facility depending on complexity to:

reduce the chance for preventable readmission

verify that the discharge instructions are clear and understood

ensure the necessary medications are prescribed available

coordinate follow up appointments

order supportive care such as home health services or PCA services
-
prevent return trips to the emergency department whenever possible
