Dr. Cook and the care team, under his supervision, collaborate with the patient via telephone or the patient portal to prevent further sickness and improve outcomes for patients with chronic conditions such as:

  • high blood pressure

  • high cholesterol

  • diabetes

  • COPD

  • asthma

  • seizure disorder

  • depression

  • anxiety

  • obesity

  • etc.

Dr. Cook sees recently discharged patients within 1-2 weeks of discharge from a hospital or rehabilitation facility depending on complexity to:

  • reduce the chance for preventable readmission 

  • verify that the discharge instructions are clear and understood

  • ensure the necessary medications are prescribed available

  • coordinate follow up appointments

  • order supportive care such as home health services or PCA services

  • prevent return trips to the emergency department whenever possible
