Nurse Practitioner | Medical Director

Dr. Wesley E. Cook is an advanced practice registered nurse (APRN) who is board-certified in primary care across the lifespan.  His clinical focus is on the care of the older adult, especially the home-bound or frail geriatric patient. Trained at Johns Hopkins University and Drexel University, Dr. Cook has practiced geriatrics in the home and long-term care settings as a family nurse practitioner (FNP-BC) since 2015, bringing more than a decade of total nursing experience to his practice. 

Dr. Cook believes that the patient and the family are the center of the care plan. Scientific evidence provides the boundaries and informs the clinical management. Being a patient is a vulnerable experience, so he strives to deliver care with dignity, respect, and a sense of shared humanity.

Nurse Practitioner | Home-Based Primary Care

NP Devermont is an advanced practice registered nurse (APRN) who is board-certified in adult and geriatric primary care. She was a student of Dr. Cook's during her graduate program, and he was so impressed with her that he asked her to join the team. NP Devermont brings a wealth of nursing experience and a keen eye for identifying and addressing problems that keep patients from optimal health and good quality of life. Her clinical interests include the management of multicomplex patients (patients with 3 or more chronic conditions) and home-based primary care.
